Creating a Summary Template

  1. Go to Clinic Settings from the top navigation, click Templates in the left sidebar, then open the form you want to add a summary to
  2. Click the Summary tab
  3. Click Create Summary Template
  4. You're now in Build mode where you can compose your summary

Adding Content to Your Template

Click the + icon in the toolbar or type / to access:

  • Variables: Select fields from your form to pull in patient responses directly
  • AI blocks: Write instructions telling the AI how to process and summarize information

Writing Effective AI Prompts

When creating AI blocks, follow these tips for the best results:

  • Be specific about what the section should include and which data to summarize (e.g., "past surgical history with dates and locations")
  • Specify the format you want (paragraphs, bullet points, numbered list, table)
  • State what to exclude (e.g., "don't include recommendations, only surface facts")
  • Keep instructions clear. The more precise you are, the better the output

Renaming Templates

Click the three-dot menu next to the template title to rename it. Click Save All to save your form and all summary templates.
 

Note: If you see errors while editing, it's usually a naming issue, such as a variable that references a form field that no longer exists or a misspelled field name. Error details appear in the preview panel or below the editor. Reach out to your Thrive contact if you need help troubleshooting.
